Slice squash in half; remove seeds. Cut each half into 8 slices, approximately 1 inch thick.

In bottom of slow cooker, place 8 slices squash. Top with 2 pork chops. Repeat layers.

In bowl, combine remaining ingredients; mix well. Pour over pork chops and squash. Cover; cook on LOW setting 5 to 6 hours or until pork and squash are tender.
